A good Support Coordinator doesn't just connect you to services — they make sure those services actually fit your life, your goals and your budget.

Support Coordination is funded under your NDIS plan (Capacity Building budget) and is designed to help you get the most out of your funding. At Disability Talk, our coordinators take the time to understand what matters to you — not just what's in your plan.

Who is it for?

Support Coordination suits participants who have it funded in their plan and want help making sense of the NDIS, building their support network, or managing multiple providers. If your needs are more complex — involving mental health, justice, housing or crisis — you may benefit from Specialist Support Coordination instead.

How it's funded

Support Coordination is funded from your Capacity Building — Support Coordination budget. It does not come out of your Core Supports.

What our Support Coordinators do

  • Understand your plan

    We read your plan with you, explain what each budget is for, and make sure you know exactly what you're entitled to.

  • Connect you with providers

    We find and vet the right providers for your needs — support workers, therapists, equipment suppliers — and help you set up service agreements.

  • Coordinate across systems

    We liaise with health, housing, education and community services so everything works together, not in silos.

  • Prepare for plan reviews

    We help you track your goals, document your progress, and prepare evidence for your next NDIS review so you go in confident.

  • Resolve issues fast

    When something goes wrong — a provider falls through, a payment is delayed — we step in and sort it out.
