Specialist Support Coordination is a higher-intensity form of support coordination for participants whose needs are complex or who face significant barriers to accessing services. Our Specialist Support Coordinators are experienced in working across systems and in high-pressure situations.
Specialist Support Coordination is for participants who have it funded in their plan, typically those with psychosocial disability, complex health needs, involvement with the justice system, or who are at risk of homelessness. Your NDIA planner or LAC will determine eligibility.
Specialist Support Coordination is funded from your Capacity Building — Support Coordination budget at a higher rate than standard Support Coordination.
Crisis and complex situation management
We step in when things are unstable — housing crisis, hospital discharge, justice involvement — and work to establish a safe, sustainable support environment.
Cross-system coordination
We liaise with hospitals, mental health services, housing providers, justice agencies and community organisations on your behalf.
Risk assessment and safety planning
We identify risks, develop safety plans, and make sure every provider in your network understands their role.
Building long-term capacity
The goal is always to reduce complexity over time — building your skills, your network and your confidence so you need less intensive support.
NDIS plan reviews and appeals
We support you through reviews, internal reviews and AAT appeals — making sure your plan reflects your actual needs.
